As a member of an interdisciplinary team, the candidate will provide service both as a case manager and physiotherapist participates to meet the clients needs. He/she will work closely with a physiotherapy assistant. The Physiotherapist participates in training, education and research activities to increase knowledge and to develop clinical, analytical and technical skillsQualifications1. Current registration with the College of Physiotherapists of Ontario
2. Experience in educating patients/families and in case management
3. Demonstrated excellence in communication, interpersonal, team work and problem solving skills
4. Knowledge of community resources
5. Evidence of client centered approach to care
6. ADP authorizer status preferred
