Visit us today and check out to see why more people from across the globe are moving here.About the OpportunityprideHealth is a Primary Health Care initiative that works to improve access to health services which are respectful, coordinated, comprehensive, and culturally safe for members of 2SLGBTQIA+ communities. prideHealth works in collaboration with other services and initiatives within Primary Health Care and portfolio partners across Nova Scotia Health to offer programming and support.Reporting to the PHC Manager, the prideHealth Community Liaison will provide support to advance the strategic priorities in the Primary Health Care, Chronic Disease Management, and Department of Family Practice (DFP) portfolios specifically related to the health and wellness of people of the 2SLGBTQIA+ community. The prideHealth Community Liaison will have responsibility for coordinating the planning, ongoing development and implementation of programs, services and initiatives assigned through leadership in the NSH Provincial prideHealth portfolio.The prideHealth Community Liaison will work with 2SLGBTQIA+ community leaders and community. Liaise with health care providers to support change management strategies that are inherent in the services plan for primary health care. The prideHealth Community Liaison has a responsibility for supporting the overall coordination of the planning, development and implementation of the activities assigned under the direction of the leadership team. The prideHealth Community Liaison is responsible for supporting the ongoing development and implementation of the provincial prideHealth quality program. The prideHealth Community Liaison activities are guided by PHC priority
areas, multiple partners and by community needs therefore, requires a professional approach. This work requires recognition and application of best practices and application of evidence informed decision making. The prideHealth Community Liaison serves as a resource to other team members in Primary Health Care provincially and the Department of Family Practice throughout NSH, as required.Travel within the catchment area is required.About YouWe would love to hear from you if you have the following:
- Baccalaureate degree preferably from a health discipline/health related science
- Current registration with professional association if applicable
- Two years' of recent related experience
- Proficiency in Microsoft Office
- Proficiency in cultural competencies
- Experience working with 2SLGBTQIA+ communities is considered an asset
- Lived experience in the 2SLGBTQIA+ community is a key requirement for this role
- Coordination and Project Management Skills
- Broad knowledge of the health system with high level understanding of population health approaches, community development, primary health care, health prevention and promotion, and disease management
- Experience in facilitation including coaching, facilitation, change management strategies, and quality improvement is an asset
- Experience working in a family practice setting with family physicians an asset
- Organizational and process management skills
- Superior time management skills in order to prioritize and complete assignments
- Sound knowledge and skill working in an interdisciplinary setting
- Ability to analyze information to identify priorities and emerging issues
- Ability to think critically, quickly and accurately; creative problem-solving ability
- Demonstrated leadership in facilitation and multiple stakeholders and service providers
- Proven ability to build, enhance, foster and maintain effective relationships
- Superior communication skills - written, oral, presentation
- Demonstrated high degree of independent decision making, self-direction, motivation and team building/leadership skills
- Ability to effectively organize heavy workload with multiple competing priorities
- Ability to function independently as well as a team member
- Must have a high level of judgment in order to make independent decisions related to responsibilities
- Valid Nova Scotia Driver's License and daily access to a vehicle
- Competencies in other languages an asset, French preferred
- Permanent full-time position; 75 hours bi-weekly
- Shifts may include evenings, weekends and holidays
- Site and location will be determined based on the successful candidate, once identified
