- Meeting clients' personal needs according to an individualized plan of service
- References and communicates updates to individualized service plans
- Performs all care tasks assigned as part of daily job routines to meet client needs
- Assists client in completing personal care (including bathing, mouth care, etc.)
- Assists client in light housekeeping and meal preparation
- Assists client in attending mealtimes and provide cueing assistance when needed
- Completes basic laundry services for clients
- Assists clients in attending and participating in Recreational
- Accompanies clients to outside appointments (i.e. medical, grooming, shopping etc.) using an approved transportation method
- Adheres to recommendations made by Parkland at Home and nursing
- Ensures supplies and equipment are properly utilized
- Documents all completed care tasks assigned as part of daily job routines
- Documents and communicates all changes in clients' health to the Parkland at Home office and if applicable, nursing staff
- Ensures supply and equipment needs are properly communicated to the Parkland at Home office and if applicable, nursing staff
- Clearly communicates expectations related to safety in the workplace
- Ensures client safety by following and applying all standard operational procedures, safe work practices and training provided
- Graduate grade 12 or equivalent
- Previous experience working with the elderly in the greater community or senior's living
- Having the knowledge and ability to use computers and information technology to improve efficiency
- Experience with Dementia and Alzheimer's considered an asset
- Proof of valid First Aid & CPR considered an asset
- Proof of valid of the Food Handlers Certification considered an asset
- Valid Driver's license an familiarity with vehicle maintenance practices required.
