Hours: 37.5 Hours per Week
Shifts: Monday to Friday, Weekends, including On-Call

The Pharmacist will be responsible for providing patient-focused care at Toronto Rehabilitation Institute (TRI) and the UHN Reactivation Care Centre. Programs include the Reactivation Care Centre at Buttonwood; spinal cord injury rehab at Lyndhurst Centre; complex continuing care, low intensity therapeutic enhancement rehab, geriatric and dialysis rehab, progressive rehab, and transitional care at Bickle Centre; stroke and brain injury rehab, geriatric rehab, musculoskeletal and multi-system rehab, and specialized dementia rehab at University Centre.Duties
- Provides patient-focused care to improve patient outcomes; this includes identifying, resolving, and preventing drug-related problems by conducting thorough pharmacotherapy assessments, performing medication histories and medication reconciliation, providing drug information to patients and healthcare teams, and other patient care activities
- Supports operational patient care activities including facilitating drug distribution and/or access, procurement, and drug coverage; promotes medication safety
- Participates in education activities including professional development and continuing education; mentorship and training of new pharmacy staff and learners
- Participates in development, review, implementation, and evaluation of projects and initiatives in line with the priorities and/or objectives of the pharmacy department and/or organization
- To provide drug information services to members of the healthcare team to promote the safe, effective, and cost-effective use of medication therapy in the patient care setting
- Effectively communicates with patients, colleagues, allied health professionals, and other members of the patient-care team
- Performs other duties consistent with the job classification, as required.
- Possess a Bachelor of Science degree in Pharmacy, Doctor of Pharmacy degree or equivalent required
- Certificate of competency from the Pharmacy Examination Board of Canada required
- Active registration with the Ontario College of Pharmacists (Part A) required
- Hospital pharmacy practice experience preferred
- Clinical experience with patient-centered care in rehab an asset
- Hospital pharmacy residency certificate an asset
- Membership in the Canadian Society of Healthcare-Systems Pharmacy an asset
- Excellent verbal and written communication skills
- Excellent interpersonal skills
- Excellent organizational and time management skills
