- Maintain the Standard of Practice Requirements in the Clinical, Interpersonal and Safety Aspects of Patient Care including:
- Assessment and Analysis: Document relevant history, clinical examination findings, problems and goals, and treatment plan.
- Planning: In consultation with the patient and their family (and with collaboration of other members of the health care team), develop an individualized, outcome orientated treatment plan.
- Implementation: Following clients consent, implement the treatment plan, and provide instruction in self-management and health promotion.
- Ongoing Evaluation: Evaluate the effectiveness of interventions and service; provide a high level of quality care through quality monitoring, program evaluation, and performance evaluation.
- Reporting: Provide timely, accurate and objective information about treatment, progress and discharge planning.
- Provide patient and family centered care.
- Ensure patient safety and adherence to infection prevention and control measures.
- Educate patients and families about his/her condition, treatment program, and self-management.
- Act as a resource to other members of the health care team.
- Participate in in-service and community education programs.
- Record statistics in accordance with Rehabilitation Services Policies and Procedures.
- Participate in the development, implementation, and evaluation of programs undertaken by the Patient Services Team.
- Coordinate the delivery of physiotherapy with services provided by other team members.
- Keep informed and remain objective of the roles of team members and their relationship to one another.
- Develop and improve own clinical competence.
- Comply with the Standards and Code of Ethics of the College of Physiotherapists of Ontario.
- Voluntarily attend in-service presentations and participate on hospital committees as requested.
- Keep informed of quality best practices in physiotherapy and health care.
- Participate in quality improvement activities to facilitate changes in care and systems.
- Performs duties in a manner that exemplifies and facilitates both quality and patient safety
- Graduate of an accredited Physiotherapy Program.
- Possess a current Independent Practice Certificate or Supervised License with the College of Physiotherapists of Ontario.
- Eligible for membership in the Canadian Physiotherapy Association.
- Possesses a current Basic Cardiac Life Support Certificate (BCLS)
- Experience treating patient with musculoskeletal, neurological and/or respiratory problems.
- Excellent interpersonal and communication skills.
- Strong computer skills particularly with Microsoft Office and hospital Software.
- Demonstrated verbal and written communications skills.
- Strong interpersonal communication skills and customer service experience.
- Strong organizational skills and ability to work in fast paced environment.
- Demonstrated ability to attend work on a regular basis.
- Demonstrated ability to maintain privacy and confidentiality.
- Demonstrated initiative and self-direction with the ability to work with minimal supervision and contribute to a team.
- Is knowledgeable of the roles of other team members and recognizes their contributions.
- Salary per the OPSEU collective agreement ($40.36 per hour to $56.30 per hour)
