- Provision of SWK services to patients and/or their families in the Cipriano Centre for Senior’s Health at an advanced level of practice and within an interprofessional team. This includes psycho-social assessments, psycho-education, counselling to patients and/or their families, advocacy, team consultation, system navigation and linkage to community resources.
- Member of interprofessional health care team leading, developing, and implementing comprehensive plan of care for patients and their families.
- Provide consultation to health care professionals and patients/families regarding geriatric issues.
- Be a role model for exemplary senior friendly care best practices
- Collaboratively exploring additional standardized tools, processes and guidelines to support integration of practice across the Cipriano Centre.
- Foster a One Team approach, ensuring integration, collaboration and navigation are embedded across all services.
- Commitment to Student Education at minimum every 2 years
- Other duties as assigned related to departmental tasks.
- Master’s Degree in Social Work required
- Member in good standing of the Ontario College of Social Workers and Social Service Workers (OCSWSSW)
- Minimum three (3) years’ experience working in a Hospital setting and/or outpatient Ambulatory care setting, and/or community healthcare team.
- Excellence and demonstrated competency in the full scope of SWK practice
- Good understanding working in Specialized Geriatric services and/or with seniors and their families is strongly preferred.
- Knowledge of Comprehensive Geriatric Assessments (CGA) is an asset.
- Experience working with elderly patients with frailty and complex medical and social needs is essential.
- Commitment to person and family centred care.
- Knowledge of legislation related to health care context of practice.
- Experience in facilitating Advanced Care Planning (ACP) and participating in Goals of Care (GOC) conversations.
- Well-established knowledge of relevant health care system processes, Regional Geriatric Program and community resources required.
- Excellent assessment, interviewing, counselling and problem solving skills.
- Strong collaborative interprofessional team skills, negotiation and conflict resolution skills.
- Comfort in providing public presentations representing the team and/or program.
- Proven well-developed organizational and time management skills; flexible and adaptable to change and adjusting priorities.
- Excellent interpersonal and communication skills (oral, written and comprehension).
- Knowledge and experience in group work dynamics and facilitating groups (i.e. support, educational, etc.) through various platforms (i.e. in-person, virtual) is an asset.
- Experience and commitment to quality improvement initiatives.
