By listening to patients and partners, we strive to connect communities to the right care, at the right place, and at the right time.JOB SUMMARY:Acute Care Nurse Practitioner accountable for the development, implementation and supporting delivery of Medical Assistance in Dying services (MAID) for WRHN’s clinical programs and supporting oncology clinical programs. The MAID Coordination aspect of the role supports health care providers, patients and family caregivers to access appropriate services and resources based on current legislative protocols and program specific best practice, and in accordance with the clinical and ethical standards of WRHN. Working autonomously and in a collaborative practice model with Ontario Health@Home, physicians, professional practice, advanced practice colleagues, patient relations and clinical leadership at all levels, the Acute Care Nurse Practitioner will coordinate all aspects of the MAID process including navigation of requests from internal and external sources, completing MAID assessments, provision of MAID, assisting with documentation and reporting requirements both provincially and federally, and developing and maintaining a robust repository of education resources. The Acute Care Nurse Practitioner will also work closely with regional partners to ensure align with best practices for provision of MAID in the wider community.The Acute Care Nurse Practitioner will work collaboratively with the Oncology NPs and Oncology leadership to provide support for evolving clinical practice at the WRHN Cancer Centre. This may include providing additional support for existing NP clinics/program or development of an independent NP led clinic supporting in an area of increased clinical demand in the oncology program. The primary focus will be on MAID services at WRHN with additional clinical work in the oncology program.QUALIFICATIONS:Required Qualifications:
  • Must be registered with the College of Nurses of Ontario in the Extended Class (EC). Consideration may be given to candidates who are eligible to write Extended Class exam with the understanding standing they must be registered in the Extended Class prior to commencing employment in this position.
  • Completion of an accredited controlled substance course according to College of Nurses of Ontario standard
  • Minimum of 5 years demonstrated experience in a health care setting with a broad knowledge base in health systems navigation
  • Demonstrated knowledge of Medical Assistance in Dying in Canada and Ontario
  • Completion of (or be willing to complete during orientation) the Canadian MAID Curriculum through CAMAP
  • Demonstrated ability to work collaboratively and assertively within an inter professional health care team and with community partners
  • Well-developed communication, interpersonal skills as well as negotiation, consultation and client advocacy skills that keep the patient at the centre of the work
  • Demonstrated ability to support patients, families and staff through crisis situations
  • Demonstrated excellent written and verbal communication skills
  • Demonstrated ability to build effective working relationships and teams
  • Ability to be self-directed and work autonomously
  • Ability to function within a collaborative practice model
  • Ability to promote professional education and advances knowledge through research and scholarly activities
  • Basic Cardiac Life Support required. Advanced Cardiac Life Support an asset
Preferred Qualifications:
  • Training and experience in project management
  • Crucial Conversations or equivalent
  • Understanding of communication styles and navigating difficult communication
  • Canadian Nursing Association Certification in Hospice Palliative Care or relevant specialty certification preferred
  • Post-graduate certificate and experience in palliative care
  • 3 to 5 years’ experience in working with MAID, hospice, oncology or palliative care
