Job DescriptionSocial Worker MSW - Chronic Disease Wellness CentreThe Social Worker is an integral member of the Chronic Disease Wellness Centre’s interprofessional care team. Cardiovascular Pulmonary Rehab patients include those with cardiac, stroke, pulmonary or lifestyle risk factors. Diabetes Education patients include adults at risk for diabetes, with prediabetes; type 1 diabetes, type 2 diabetes, gestational diabetes and young adults transitioning from pediatric programs - from diagnosis throughout their self-management journey. The Social Worker will assess patients and intervene using psychosocial assessment strategies and therapeutic interventions with individuals, groups and family members. Strategies, to help patients achieve optimum psychosocial functioning and self-management, include counseling, Cognitive Behaviour Therapy, Motivational Interviewing, goal setting and mindfulness techniques and strategies.Contribute to safe, quality care by
  • Meeting with patients to assess their psychosocial needs and intervene in a way that supports and empowers them to make healthy lifestyle changes, increase self-efficacy and develop self-management skills
  • Providing counseling, problem-focused support, social information, support tools and connect patients with resources
  • Developing and facilitating support groups and therapeutic education sessions
  • Developing and maintaining community resource linkages that support patients/families
  • Working collaboratively with team members and support them as they provide care to patients
What must you have?
  • Current registration with the Ontario College of Social Workers and Social Services Workers,
  • Master’s degree in Social Work from an accredited University,
  • Recent counseling/therapy experience within a Cardiovascular Pulmonary Rehab and/or Diabetes Education Program or other outpatient setting (minimum 3 years preferred).
  • Proficiency in current, evidenced-based methods and practices of social work including psychosocial assessment, consultation, support and treatment for individuals living with chronic health.
  • Understanding of social determinants of health and their impact on health and well-being
  • Demonstrated experience with case management, system navigation and advocacy
  • Strong knowledge and familiarity with community support services including local mental health supports, financial assistance programs (Ontario Disability Support Program, Ontario Works, Canada Pension Plan Disability), medication and diabetes supplies coverage programs, recreation and exercise facilities, mobility and transportation services and homecare
