Reporting to the Executive Directors of Knollcrest in Milverton and Greenwood Court in Stratford, the Registered Nurse with Extended Class: RN(EC) privileges (Herein referred to as the Nurse Practitioner (NP), is a management position with the role of practitioner, consultant, educator and advocate. The RN(EC) practices autonomously, offering the full scope of primary health care nurse practitioner practice. He/she functions as a member of the interdisciplinary care team in the development, implementation and evaluation of the resident's plan of care.The Nurse Practitioner collaborates with the Medical Director, other physicians and community services and agencies in the provision of primary health care for residents residing in the Homes as well as supporting more broadly the services of Knollcrest and Greenwood Court.The Nurse Practitioner collaborates with the Directors of Care and multi-disciplinary teams in the provision and advancement of nursing services, in the development, implementation and evaluation of the Nursing department.Role Responsibilities:
  • Performs the responsibilities of the position within the legislative and regulatory standards set out in the applicable Provincial Acts. Performs the responsibilities of the position consistent with the Operational policies of Knollcrest and Greenwood Court.
  • · Working within the legislated scope of practice of the Nurse Practitioner, including diagnosing, prescribing, ordering, and interpreting lab test/x-rays/ultrasound as per regulations. Provides direct comprehensive care to residents with specialized and complex needs.
  • · Collaborates and consults with the multidisciplinary team to ensure comprehensive assessment, analysis, treatment planning and evaluation of resident's care.
  • · Consults with and refers to other health care professionals and community services as needed.
  • · Provides medical coverage on request of the Medical Directors and other physician staff.
  • · As a consultant, the RN(EC), assists, collaborates with, guides and supports staff with challenging clinical situations, provides nursing consultation for clinical policy and practice guideline development and implementation, collaborates with Management in the development, implementation and evaluation of clinical policy and practice guidelines. Participates in professional nursing associations and other external agencies to further the development of nursing and provision of services to residents within the Home.
  • · As an Educator, the RN(EC), provides ongoing information to staff, resident and family, fostering involvement, choice and autonomy, and contributes to the planning, initiating, implementing and evaluation of educational programs and learning resources for residents, family, staff and community. The Nurse Practitioner participates in the identification, analysis, and interpretation of trends in client outcomes and professional nursing practice issues in order to determine priorities for educational programming for residents, family, staff, and community. The Nurse Practitioner collaborates with the Director of Care in providing educational training on advanced nursing practice issues including new equipment. Provides preceptorship for students, especially student nurses practitioners.
  • · As an Advocate, the RN(EC), promotes evidence-based primary health care services and nursing care of older adults by collaborating with the Medical Directors, medical staff, RN's, and appropriate external agencies. Counsels and supports residents and substitute decision-makers and families regarding advanced care planning and consent to treatment issues. Participates in the identification, analysis, and interpretation of trends in care in order to assist the Homes, in collaboration with the residents and their caregivers, to determine and address health care priorities and related issues. Participates in pertinent evaluation and research studies related to the care of older adults and primary health care nurse practitioner programs.
  • · Participates in Management meeting on request, Professional Practice Committees, Pharmacy and Therapeutics Committees and appropriate Committees as required.
  • · Liaises with community services and agencies to ensure appropriate utilization of specialized services in the Homes.
  • · Protects own health and health of others by adopting safe work practices, reporting unsafe conditions immediately, and attending all relevant in-services regarding occupational health and safety. Attends all relevant in-services regarding occupational health and safety. Follows all guidelines for employees and employers as legislated under the Ontario Occupational Health and Safety Act.

  • Collaborates with MD partners as required, including but not limited to requiring medication orders for medications and diagnostic testing not in NP scope
  • Promotes and uses clinical guidelines and evidence-based practice in provision of health care to residents in a long-term care setting
  • Maintains confidentiality of information as outlined in the policies of sponsoring agency and the privacy legislation
  • Manage care processes that enable each team member to deliver high quality care and achieve excellence in professional practice ensuring compliance with regulatory standards
  • Provide accessible and flexible leadership in fast paced setting
Technical and Clinical Management:
  • Guide development of and approve policies, processes, and procedures
  • Manage equipment maintenance programs
Financial Management:
  • Budget adherence and analyze variances
Patient/Resident and Staff Safety and Quality Risk Management:
  • Develop/implement quality activities to maximize service and minimize patient/resident risk
  • Review and analyze data for trends, coordinate quality improvement initiatives and guide corrective actions
  • Responsible for providing a safe working environment for staff and supporting safety initiatives in collaboration with the Occupational Health and Safety department & Clinical Services Team
Clinical Responsibilities:Health Assessment & Diagnosis:
  • Demonstrated competency in: health assessment and diagnosis; health care management and therapeutic intervention; health promotion and prevention of illness, injury, and complications in the care of older adults
  • Advanced knowledge, skill, decision making, and expertise in the nursing care of complex frail older adults
  • Advanced clinical knowledge and a strong commitment to promoting professional practice and evidenced based resident centered care
Therapeutic Management & Performing & Ordering Procedures:
  • Demonstrates the competencies to autonomously diagnose, order and interpret diagnostic tests, prescribe pharmaceuticals, and perform procedures within the RN(EC) legislated scope of practice and designated medical directives as appropriate
Ethical Practice:
  • Advocates for evidence-based quality health care practice
  • Supports team members, LTC residents and their families and staff in LTC to advocate for the appropriate resource and service as health care needs require
  • Advocating for the need of additional health care provision for residents in LTC
Interprofessional & Collaborative Care:
  • Ongoing consultation and collaboration with interprofessional team members including the consulting MD when planning and implementing care
  • Develops and maintains collaborative working relationships with members of the health care team within the LTC as well as within the acute care hospital (i.e. GEM Nurses) and within the community.

Qualifications:
  • Current registration with the College of Nurses of Ontario as an RN (EC), Nurse Practitioner (unrestricted)
  • Working knowledge of College of Nurses of Ontario standards of practice
  • Excellent communication, leadership, and problem-solving skills; emotional intelligence and strong communication skills are essential
  • Effective program development, evaluation, resource allocation and project management skills
  • Demonstrated current expertise in geriatric care & adult chronic disease management
  • Proven ability to establish positive relationships, team building
  • Ability to be flexible and adapt to the challenge of a busy environment
  • Management of inter-professional teams with effective team building skills, analytical and problem-solving skills
  • Self-directed, works independently, and is able to achieve outcomes
  • Proven success in implementation of strategic program goals including quality indicators and volume targets
  • Track record of innovative, challenging, and successful achievements
  • Excellent report writing skills
  • Ability to create a vision for the development of comprehensive programs related to the various services provided now and, in the future
  • Demonstrated good attendance record
  • Valid drivers' license and a vehicle required
