This position is based within the Home Care branch of Continuing Care that offers a range of services tailored to meet individual needs, including acute care, rehabilitation, and support for chronic and palliative conditions. These services are determined through personalized assessments to help clients remain in their homes, providing respite for caregivers and promoting independence and a higher quality of life. Every action and decision is guided by Continuing Care's Vision: 'Exceptional care, honoring your culture and traditions” Mission: “Together, we offer person-centered care that nurtures well-being” and Values: “Connectedness, Dignity, Equity, Integrity and Respect” The position contributes to an environment that is inclusive, non-discriminatory, and is physically, psychologically and culturally safe.Your roleThis position is responsible for preforming assessments, evaluating care requirements, and assigning clients requiring Continuing Care Services, including home care and long-term care services to appropriate staff for client care navigation. You are an essential point of contact and collaborative partner, for stakeholders such as the Yukon Hospital Corporation, to support client discharges to Continuing Care programs, while supporting an Aging in Place and Home First Philosophy.

  • Eligible for Registered Nurse registration with a Canadian registered nursing regulatory authority
  • Recent experience in screening, intake, and coordinating services for clients with complex health conditions transferring to a long-term care/home care setting
  • Experience providing advanced nursing assessments within the scope of a Registered Nurse

  • Valid class 5 driver's licence
  • CPR Level C with AED (completed within the past 30 months)

  • Current registration with the Yukon Registered Nurses Association
