The Manager leads with a patient-centred, evidence-based approach, supporting professional practice and fostering high-quality care. This role also requires strong fiscal and business acumen and serves as a role model for Southlake's core values and competencies. The portfolio includes services for diverse populations such as long-term care residents, community-based palliative patients, and individuals accessing non-bedded hospital services.Primary ResponsibilitiesPatient Care Responsibilities
- Drive the planning, implementation, and evaluation of evidence-based, outcome-focused improvements in clinical practices and care delivery models, including quality improvement initiatives, workflow redesign, and the adoption of innovative technologies.
- Foster strategic partnerships with funders to explore and advance opportunities for program growth and service expansion across Community Program Services.
- Coordinate and oversee clinical and non-clinical operations, with a focus on Community Services clinics located outside the main hospital campus, ensuring consistent standards of care and service delivery.
- Lead the introduction and integration of new technologies, products, and procedures, ensuring alignment with current policies and best practices in patient care.
- Proactively address and support the resolution of concerns raised by patients, physicians, and staff to promote a culture of service excellence and continuous improvement.
- Clearly communicate performance expectations to all interdisciplinary team members, fostering accountability and alignment with organizational goals.
- Apply the performance development model to monitor, assess, and evaluate individual and team effectiveness in meeting patient care needs.
- Lead all aspects of staffing, including recruitment, interviewing, hiring, disciplinary actions,
- Oversee staff scheduling, attendance management, payroll processes, and work assignments to ensure operational efficiency and compliance.
- Ensure comprehensive orientation and onboarding for all new interdisciplinary staff, supporting their successful integration into the program and broader organization.
- Contribute to the development, oversight, and analysis of both operational and capital budgets
- Support the collection, analysis, and maintenance of statistical data to meet reporting requirements and inform budgetary decisions.
- Lead and support the development, implementation, and evaluation of patient care guidelines across clinical and community-based settings, ensuring alignment with best practices and quality standards.
- Oversee program planning, execution, and evaluation within Community Services, driving service excellence and responsiveness to community needs.
- Develop, implement, and evaluate evidence-based practice initiatives and educational resources to support the ongoing development of clinical staff.
- Participate in, support, and lead research activities related to the clinical area, contributing to knowledge advancement and integration of best practices
- Actively participate in internal and external committees, as assigned by the Director, to support organizational priorities and collaboration across departments.
- Provide support to other leadership roles as needed
- Participate in the on-call rotation to provide managerial support.
- Perform cross-functional responsibilities and other duties as assigned, in alignment with the scope and classification of the role.
- Bachelor's degree in a registered health profession, master's degree preferred
- Registration with the appropriate College/Regulatory body
- Minimum of 5 years' formal people leadership experience
- Minimum of five (5) years of progressive leadership experience, ideally in community and clinic-based healthcare settings, including experience leading teams in unionized environments.

- Demonstrated ability to lead through change using formal change management strategies and a people-first approach.
- Strong analytical, critical thinking, and decision-making skills to guide service delivery, planning, and problem-solving.
- Experience in quality improvement, research, or service design initiatives that improve patient and team outcomes.
- Comprehensive understanding and application of the Regulated Health Professions Act and relevant health legislation.
- Actively engaged in committee work and stakeholder collaboration, with a focus on implementing and sustaining meaningful outcomes.
