Our interdisciplinary team supports individuals with complex medical needs, helping them regain independence and thrive in their own environments.💼 Position SummaryAs an Occupational Therapist, you’ll play a vital role in helping clients achieve and maintain their highest level of functional independence. You’ll assess, plan, and implement personalized rehabilitation programs, working closely with clients, families, and healthcare teams to ensure safe, effective, and meaningful care.This role is ideal for a compassionate, innovative, and collaborative professional who thrives in a dynamic home and community care setting.🧠 What You’ll DoClinical Responsibilities:
- Conduct holistic assessments of cognitive, physical, perceptual, and emotional function
- Develop and implement individualized treatment plans
- Educate clients and caregivers on therapy goals, exercises, and adaptive equipment
- Collaborate with interdisciplinary teams for discharge planning and care coordination
- Supervise OTA/PTA staff and mentor students as needed
- Support clients with complex medical conditions in their homes
- Follow infection control protocols and ensure safe use of equipment
- Promote client safety and participate in safety audits and initiatives
- Maintain clean and organized therapy spaces
- Document care activities and maintain accurate client records
- Assist with scheduling, referrals, and program development
- Participate in quality improvement and professional development initiatives
- Bachelor’s or Master’s Degree in Occupational Therapy (Master’s preferred)
- Registered with the College of Occupational Therapists of Ontario (COTO)
- Current BCLS/HCP certification
- Membership in OSOT/CAOT (preferred)
- 1–3 years of clinical experience (required)
- Experience with complex medical conditions and mobility support (required)
- Home and community care experience (preferred)
- Familiarity with mobile work tools and EMR systems (e.g., Procura, HPG)
- Strong communication and interpersonal skills
- Creative problem-solving and critical thinking abilities
