We’re looking for a skilled and compassionate Social Service Workers to join our Integrated Care Solutions team.Person-and family-centered care (PFCC)Person-and family-centered care (PFCC) is an approach to care that recognizes the importance of family in a patient’s life and the importance of their active involvement in planning and making decisions on health care, services and treatment, and health system reform. This approach to care provides respectful, compassionate, culturally safe, and responsive care that meets the needs, values, beliefs, and preferences of the patient, their family, and others identified as significant to their life from diverse backgrounds and settingsPart-time Position Available: Requires at least 2 days a week of daytime/weekday availabilityHome Care Position: pays a per visit rate and mileage in-between clientsAreas of Service: Niagara Falls, St. Catherine's and WellandDuties and Responsibilities
  • Implement Care Plans: Execute social and wellness care plans under the supervision of a Licensed Social Worker. Provide in-person and virtual support for wellness checks, food and housing security, and social connections.
  • Community and Resource Management: Follow up on community service referrals, manage patient transportation, and advise clients on available resources. Facilitate referrals and assist in discharge planning when social work intervention is no longer needed.
  • Caseload and Record Management: Efficiently manage caseloads and maintain treatment records in compliance with the Ontario College of Certified Social Workers and Social Service Workers standards.
  • Client Support and Education: Use a client-centered approach to address psychosocial issues, provide relevant educational resources, and participate in quality improvement initiatives.
  • Professional Development and Collaboration: Engage in ongoing professional development, maintain confidentiality, and build positive relationships within the interdisciplinary healthcare team. Adapt to a dynamic environment and perform additional tasks as assigned.
QualificationsQualifications and RequirementsPosses a diploma in Social Service Work or equivalentBe an active member in good standing of the Ontario College of Certified Social Workers and Social Service Workers
  • Possess general computer skills and have access to a computer with appropriate password protection and internet capabilities
  • Minimum of two years of community-based therapy experience (preferred)
Be able to drive, hold a current, valid driver’s license and have access to a vehicle in good operating condition (preferred)Provide a police clearance (Vulnerable Sector Check) that meets the Bayshore Home Health policyProvide TB proof that meets the Bayshore Home Health policyHold a current CPR certificate (CPR-C or BLS and is only acceptable a year after issue)As Canada’s largest and best homecare company, join us as we continue our 50+ year tradition of supporting our Social Workers to make “being the difference” a Career!Bayshore is committed to a high quality and safe environment for both our staff and clients/patients.
