- Current registration (in good standing) with College of Nurses of Ontario •
- Applicants with current dialysis experience in acute and chronic settings will be given first consideration – Hemodialysis and Peritoneal Dialysis.
- Minimum of two (2) years medical/surgical experience preferred
- BCLS • Demonstrated successful completion of Home Hemodialysis Renal Orientation.
- Demonstrated assessment skills and the ability to function independently • Demonstrated good interpersonal and communication skills with an ability to work with a multidisciplinary team, including other health team members, patients and family members.
- Demonstrated ability to work autonomously and within a team environment.
- Responsible, energetic individual with above average ability to prioritize and organize in the home setting.
- Demonstrated positive relationships with other health care professionals and able to contribute to the development of an environment conducive to excellent patient care
- Ability to respond to emergency situations by rapidly identifying priorities and complex patient needs
- Ability to prioritize and complete assignments in a reasonable length of time
- Demonstrated evidence of recent continuing education
- Demonstrated understanding of and commitment to Grand River’s comprehensive safety programs and practices, including staff, patient, and environmental safety, and participation in regular in-service education in this area
- Must have own reliable vehicle and valid driver’s license for home visits.
- Good attendance record
