- Serve as the central clinical point of contact for inquiries related to BSSP/BSO, VBM, and Ambulatory Services.
- Support health care partners and family members in completing referrals and navigating clinical pathways.
- Conduct screening and triage using established BSO referral processes and geriatric assessment tools.
- Provide clinical expertise in managing responsive behaviours and cognitive impairment, ensuring client- and family-centered care from intake to discharge.
- Lead or support complex care resolution tables and cross-sectoral transition planning.
- Facilitate the development and sharing of educational resources across BSSP/BSO, VBM, and Central Navigation.
- Maintain and update content for the BSSP/BSO mini website and contribute to Central Navigation resources.
- Coordinate alignment and pathways between LTC Leads, BSO Outreach, Acute Care, Addictions Specialists, and Caregiver Supports.
- Support Central Navigation functions including referral triage, early clinical contact, and standardized prioritization.
- Collaborate with Quality and Business Intelligence teams to monitor key access indicators (e.g., first contact, wait times).
- Master’s Degree in a regulated healthcare profession (e.g., Social Work, Occupational Therapy, Nursing).
- Current registration/licensure in good standing with a professional regulatory body.
- Minimum three (3) years of experience in geriatrics, community-based seniors care, and managing responsive behaviours.
- Experience with BSO systems, ambulatory services, and navigation preferred.
- Familiarity with RAI-HC, cognitive assessments, caregiver burden scales, and geriatric assessment tools.
- Completion of or willingness to complete GPA, P.I.E.C.E.S., Montessori for Dementia Programming is an asset.
- Strong clinical reasoning, critical thinking, and problem-solving skills.
- Experience providing caregiver education, support, and counselling related to caring for individuals exhibiting responsive behaviours.
- ·Demonstrated experience collaborating with primary care providers and specialists in the care of older adults exhibiting responsive behaviours.
- ·Proficiency in Meditech, Outlook, Adobe, and other relevant applications.
