The Peter Munk Cardiac Triage office provides resources designed to improve the healthcare of patients with new and on-going cardiac conditions. The Nursing Clinical Coordinator, in collaboration with the healthcare team, will facilitate patient care, improve the education of the patient and work collaboratively with internal and external healthcare providers to schedule patients for various procedures in the Cardiovascular Investigations Unit.DutiesAs a member of the Cardiac Triage office, the responsibilities of the Clinical Coordinator, include:
- Obtains/reviews/triages medical records
- Schedules patients for various procedures in the cardiovascular investigations unit
- Provides education and support to patients and family members
- Utilizing and applying current specialized knowledge and expertise on recent advances in nursing practice to facilitate the effective management of clinical activities
- Coordination of patient care for inpatients and external referrals
- Performs cross coverage of other Nursing Clinical Coordinator roles and duties as needed
- Participating actively in Quality Improvement initiatives
- Participating in academic and research activities/projects
- Current registration with the College of Nurses of Ontario
- Bachelor of Science in Nursing required
- Current BCLS Certification
- Minimum five (5) years Cardiac +/or Cardiac Surgery experience required
- Critical Care or Coronary Care Certification preferred
- Ambulatory experience preferred
- Experience working with community health providers preferred
- Experience in change management
- Strong organizational and time management skills

- Excellent verbal and written communication skills
- Demonstrated ability to cultivate and build relationships
- Strong track record of high- quality work and engagement is essential
