
Pharmacist

- Responsible for the provision of clinical pharmacy services for the purpose of achieving definite outcomes to improve the patient’s quality of life
- Review of physician’s orders, creation, and maintenance of patient medication profiles, and may include IV admixture, TPN, pediatric and chemotherapy preparation, drug information, drug therapy monitoring and adverse drug reaction reporting depending on the site
- Order verification
- Other duties as assigned
2. Current registration with the Ontario College of Pharmacists required – Part A of Register
3. Previous hospital experience in unit dose and/or IV admixtures preferred
4. Hospital residency program or equivalent an asset
5. Demonstrated ability to work independently and as a member of a team
6. Demonstrated ability to prioritize a consistent and varied workload
7. Ability to direct and teach staff.
8. Demonstrated written and verbal communication skills required, in order to facilitate effective communication with patients, peers, and other health care providers.
9. Demonstrated ability to identify, prevent and resolve drug related problems.
10. Active membership in the Canadian Society of Hospital Pharmacists (CSHP) is an asset.
11. Good understanding of change management; must be able to identify change needs and proactively act on those needs.
12. Leadership, problem solving, organization skills required.
13. Effective negotiation and conflict management resolution skills required.
