On-Call RotationWhat You’ll DoAs a Visiting Nurse, you’ll play a hands-on role delivering personalized high-quality care to your clients in their community. As a primary care planner to your clients, you have the autonomy and flexibility to provide a holistic approach to care while increasing your skills through our comprehensive training, mentorship and preceptorship programs.You will teach, delegate tasks and functions to different levels of clients and family members while maintaining our industry’s highest level of practice.You are the nursing care expert for your community. With our efficient work schedules, you will be focused more on care time and less travel time. Common tasks can include, but are not limited to:
- Participate in on-call rotations as required (typically twice per month overnights)
- Conduct in-home assessments and deliver nursing care based on individualized care plans
- Administer medications, wound care, IV therapy, palliative, pediatric care, and other clinical interventions
- Monitor patient progress and update care plans accordingly
- Educate patients and caregivers on disease management, medication adherence, and health promotion.
- Collaborate with interdisciplinary teams and task delegation to ensure continuity of care

- Graduate of an accredited School of Nursing (RN)
- Current registration in good standing with the provincial regulatory body (CNO)
- LEAP Training, Palliative Certification, considered an asset
- 2-5 years of recent relevant nursing experience in a fast-paced clinic or medical environment
- Experience with wound care, diabetes management, trach/vent, IV care is an asset
- Valid G2 or G Driver’s License and access to a reliable vehicle
- Willingness to travel regularly within the Hamilton/Burlington area in all weather conditions
