If you are seeking meaningful work in an organization where your contributions truly matter to patients, colleagues, and the broader community, you have come to the right place.Current Shifts: Primarily Days 8 hour shifts (0.6FTE)Position Summary:The Social Worker is an integral member of an interdisciplinary team and provides a range of services to patients and their families, including mental health and psychosocial assessments, brief solution orientated counselling, discharge planning and linkage to community resources.In accordance with the standards of the Ontario College of Social Workers and Social Service Workers and the mission, vision, values and policies of GGH, Social Workers provides high quality, safe, effective and evidence-based care in partnership with the patient and family.In this role, you will:

Qualifications, Experience, Skills and Abilities:

Master’s Degree in Social Work (MSW)
Current membership with the Ontario College of Social Workers and Social Service Workers
2-3 years experience working with a multi-disciplinary team in a health care setting
Experience with delivering specific therapeutic interventions with individuals using approaches such as cognitive behaviour therapy and solution-focused therapy
Research experience /education with bariatric population and weight loss program Student education/supervision
Demonstrated knowledge of available community resources
Excellent working knowledge of Consent and Capacity Legislation, Social Work Code of Ethics, Mental Health Legislation and the Heath Care Consent Act
Demonstrated excellent knowledge base in discharge planning
Ability to prioritize and manage multiple complex cases
Highly developed program planning and time management skills
Demonstrated leadership and problem solving abilities
Well-developed skill in risk assessment, establishing safety plans, providing brief supportive counseling and mediation
Excellent interpersonal and communication skills, both verbal and written
Demonstrated ability to work independently and in a self-directed manner as well as collaboratively within a team in a fast-paced and ever-changing environment
Demonstrated commitment to excellent customer service when interacting with patients, families, colleagues and physicians
