Job Code: 18825Status: Full TimeLocation: Providence Care HospitalUnion: OPSEU 4104 ParamedicalWage: $47.20 - $54.81Posting Period: August 14, 2025 - August 20, 2025Position Summary:This position with the Providence Care – Transitions Home Program will be engaged in community work and involves commuting across the Southeastern Ontario region. The Transitions Home Program works at achieving timely access to care and clients’ functional outcomes following discharge from PCH/PTCC. Therapeutic interventions will be carried out in the client’s home and community.The Physiotherapist, reporting to the Clinical Program Coordinator, acts as the fundamental resource to program staff, clients, caregivers and community partners. In collaboration with the program staff, the Physiotherapist provides assessment of assigned clients and develops and implements individual physiotherapy treatment programs. The Physiotherapist is accountable for maintaining the Standards of Practice and Ethics of the College of Physiotherapists of Ontario. The Physiotherapist will provide clinical education and experience for allied health students and will participate in formal teaching and research in collaboration with Queen’s University or other institutions. As a team member, the Physiotherapist participates in profession-specific and inter-professional activities that are directed towards a person-centered environment and contribute to quality, operational efficiency, professional and program development. As Physiotherapist, the goal is to help clients be healthier at home while benefitting from our supports of rehabilitative care.Skills and Qualifications Required:
  • Graduate of a recognized school of Physiotherapy
  • Active, unrestricted license to practice with the College of Physiotherapists of Ontario
  • Experience in specified area of practice preferred
  • Experience in student supervision an asset
  • Minimum of two (2) years of relevant professional experience, preferred
  • Experience working with home care teams and coordinating community services.
  • Sound knowledge of rehabilitation best practices.
  • ADP authorizer, preferred
Position Responsibilities:
  • Utilizes evidence-based practice and best practice
  • Utilizes outcome measures to guide clinical decision-making
  • Reviews medical and referral information
  • Performs appropriate evaluation/assessment
  • Analyses and states clinical findings
  • Establishes treatment goals in collaboration with the client
  • Identifies and implements treatment to achieve client goals
  • Ensures informed consent is obtained for assessment, treatment, and disclosure or release of information
