
Physiotherapist, Full Time, Transitions Home Program, RTC

- Graduate of a recognized school of Physiotherapy
- Active, unrestricted license to practice with the College of Physiotherapists of Ontario
- Experience in specified area of practice preferred
- Experience in student supervision an asset
- Minimum of two (2) years of relevant professional experience, preferred
- Experience working with home care teams and coordinating community services.
- Sound knowledge of rehabilitation best practices.
- ADP authorizer, preferred
- Utilizes evidence-based practice and best practice
- Utilizes outcome measures to guide clinical decision-making
- Reviews medical and referral information
- Performs appropriate evaluation/assessment
- Analyses and states clinical findings
- Establishes treatment goals in collaboration with the client
- Identifies and implements treatment to achieve client goals
- Ensures informed consent is obtained for assessment, treatment, and disclosure or release of information
- Specialized sub-acute Care in rehabilitation, mental health services, complex medical management and palliative care
