
Adult Day Program Clinical Resource Coordinator

- Minimum 1-3 years' experience working with individuals with Alzheimer's Disease or related dementias
- Degree or Diploma in a Health-related field such as Activation, Therapeutic Recreation, Social Service Worker, Gerontology, or Occupational/Physical Therapy Assistant
- Experience in planning and implementing therapeutic recreation programs and activities, activation methodologies, principles and practice
- Experience in providing personal grooming, bathing, toileting and nutritional assistance for clients with an irreversible dementia
- A thorough understanding and working knowledge of Alzheimer's disease and related dementias
- Trained in dementiaAbility©, Positive Approaches© to Care and/or U-FIRST/PIECES© preferred
- Demonstrated ability to manage a high volume client caseload and varied work assignments with accuracy, flexibility and in a timely fashion
- Ability to work independently under pressure and establish priorities under time constraints
- Self-motivated with demonstrated ability to function as a part of a team and/or independently, creative, flexible and exhibits a good sense of humour
- Computer literacy with developed skills in various software applications including word processing skills (i.e. Windows 2000 and MS Office), spreadsheets, databases, and presentation/graphics
- Strong organizational skills, problem-solving skills and critical thinking skills
- Excellent oral, written and interpersonal and communication skills
- Knowledge of the Home and Community Care Support Services processes and community services
- Strong sense of caring and compassion
- Experience in a community-based setting preferred
- Strong organizational and communication skills
- Knowledge and sensitivity to work in a culturally diverse environment
- Excellent past performance and attendance record.
