Provincial, BCThe Clinical Coordinator, Medication Reconciliation oversees and coordinates the Medication Reconciliation program that delivers the medication history (BPMH) and medication reconciliation for BC Cancer patients across the province. As an expert practitioner in oncology pharmacy practice, the role provides clinical leadership and education and is responsible for all aspects of the program, including quality improvement programs, supervising staff and training staff.What you’ll do
  • Develop, plan, implement, oversee, and deliver the Medication Reconciliation provincial program for BC Cancer patients across the province. Evaluate program effectiveness and make and/or recommend revisions to the program as required.
  • Act as a clinical resource in pharmaceutical care for patients on service and individualizes drug therapy through methods. Provide expert advice and guidance to Clinical Pharmacists and staff on the provincial program.
  • Supervise staff involved in delivering the provincial program. Participate in the selection, hiring and orientation process of the provincial program. Coordinate and deliver training and education to staff, pharmacy residents and students related to the provincial program. Provide feedback and escalate concerns as required.
  • Conduct medical literature research, critically evaluate and interpret clinical trials data, seeks interdisciplinary input as appropriate, and summarize and report information.
  • Perform quality assurance checks as required to ensure relevance, appropriateness, and timeliness of program delivery. Ensure the provincial program complies with Accreditation Canada requirements.
  • Maintain current knowledge in Pharmacy by methods such as reviewing literature, consultation with other health care professionals, contributing to pharmacy publications such as the Systemic Therapy Update and the Cancer Drug Manual.
What you bring
  • Post Graduate Doctor of Pharmacy degree or other related advanced degree from an accredited university.
  • Completion of an accredited Hospital Pharmacy Residency Program.
  • Licensed with College of Pharmacists of British Columbia.
  • Minimum four (4) years of recent related oncology and/or hematology experience including two (2) years in a leadership role, and/or an equivalent combination of education, training and experience.
You have:
  • Comprehensive knowledge of BPMH, medication reconciliation, medication safety principles and practices.
  • Demonstrated ability to supervise and provide technical guidance to others.
  • Demonstrated ability to teach staff and students, including clinical training and didactic teaching.
  • Broad knowledge of the applicable standards of practice and guidelines for clinical practitioners.
  • Comprehensive knowledge of hospital pharmacy computer applications, information systems, patient care information systems and related software.
  • Comprehensive knowledge of drug distribution processes in a hospital setting.
  • Demonstrated ability to represent your clinical specialty and across the continuum
  • Commitment to upholding the shared responsibility of creating lasting and meaningful reconciliation in Canada as per TRC (2015) and BC's Declaration on the Rights of Indigenous Peoples Act (2019).
  • Knowledge of social, economic, political and historical realities impacting indigenous communities and familiarity with Indigenous Cultural Safety and anti-racism and accompanying reports (BC DRIPA, TRC, etc.).
