Closing Date: April 24, 2026Position SummaryAs a member of Gattuso Centre for Social Medicine Team, the Program Lead works in support of and in good collaboration with the Social Medicine leadership team, Senior Manager, Community Health Worker Leads, clinical team and community partners to provide leadership and direction for program operations and care services. The position will support clinical activities as part of the Social Medicine services, including the implementation and evaluation of new models of care, day to day management of the services, and is a unique opportunity in the field of social medicine at a first-of-its-kind Centre.Duties
- Responsible for the day to day management and organizational effectiveness of the clinical operations, including Stabilization and Connection Centre, Mobile Health Clinics, Winter Services and Social Medicine Hub
- Provide leadership and direction to the interprofessional and administrative team and community staffing partners
- Assume responsibility for the effective oversight of operational details, patient care and quality
- Ensure program goals and objectives are aligned with the hospital's strategic business objectives
- Facilitate the interprofessional team model, ensuring that the model of care delivery is consistent with UHN mission, vision, values and strategic plans
- Facilitate the development of service delivery solutions by collaborating and/or coordinating with community partners, physicians, managers of other interprofessional teams, units/departments to ensure continuity and optimal patient care and social support
- Supports the implementation of best practices, quality documentation and accountability models
- Evaluate the patient care and social support, planning and implementation processes within the department; liaising and collaborating with the Management Team to ensure the program's patient population is provided optimal care
- Perform various corporate management responsibilities which support and advance the Social Medicine and Population Health and UHN goals
- Communicate and disseminate relevant information to staff through team meetings, in-services and other appropriate communication media (e.g. email, shared drive)
- At minimum, completion of a bachelors degree in regulated health profession or recognized equivalent required.
- At least 4-6 years of practical and related experience
- Strong knowledge of, and familiarity with the principals of population health and health equity
- Leadership or project management experience preferred
- Experience in maintaining high quality practice in the clinical and community environment
- Ability to provide duties in a professional manner and produce high quality work
- Exercise and initiative and good judgement with the ability to multi-task
- Exceptional time management, adaptability, and strong communication skills to handle shifting priorities and high-pressure situations
- Effective organizational, communication, and interpersonal skills
- Knowledge of applicable legislative, UHN and/or departmental policies
- Client service oriented, with the ability to effectively work with diversity and appreciate that people with different opinion, backgrounds and characteristics bring richness to the challenge or situation at hand.
- Membership in good standing with a Regulated Health Professional College, if applicable
