This role is ideal for a dynamic healthcare professional with strong clinical reasoning skills and a passion for discharge planning, supporting patients in achieving optimal recovery and successful reintegration into the community.The successful candidate will collaborate closely with allied health and other members of the interprofessional team to deliver exceptional, patient-centered care.QualificationsHalton Healthcare endeavors to hire individuals who are competent in their chosen profession and who exemplify our mission, vision and values of accountability, respect and compassion which guide our individual behaviours and attitudes.
  • Baccalaureate or Master's Degree in Occupational Therapy required
  • Registered and in good standing with the College of Occupational Therapists of Ontario
  • Member of the Canadian Occupational Therapy Association or Ontario Society of Occupational Therapists preferred
  • Minimum 2 years of hospital experience
  • Experience in neurological rehabilitation and with palliative patient populations is considered an asset.
  • Experience working with a variety of patient populations
  • Knowledge of evidence-based clinical practice and care pathways
  • Knowledge and experience in the use of outcome measures
  • Must be flexible and demonstrate an ability to work independently as well as collaboratively with all team members
  • Strong interpersonal, communication and leadership skills
  • Excellent organizational abilities with proven time management skills
  • Must be able to meet the physical demands of the position
  • Satisfactory employment and attendance record required
