Our Vision is to help adults and children maximize their functional abilities within their environment and enable them to participate more fully in life’s activities and rewards. We are looking for Registered Physiotherapists to provide services to our pediatric/young adult clients residing in group homes within the community. The position can be a flexible one, with working hours ranging from part-time to full time depending on your desired client caseload size. Visit us on http://www.car-rehab.comYour impactAs a Lifemark physiotherapist, you’ll be a vital member of a multidisciplinary team, empowering individuals to overcome challenges, improve their mobility, and achieve their health goals. You'll be more than a care provider; you'll be an educator, motivator, and problem-solver, lifting people up, meeting people where they are in their health journeys, sharing knowledge, and delivering top-quality, patient-centered care. In this role, you will:
  • Provide ministry funded and private clients, community-based Physiotherapy services within the pediatric/young adults client’s place of residence and/or through virtual services
  • Provide physiotherapy assessment (through interview, standardized and informal assessment techniques) in the areas of gross motor skills, co-ordination, neurological, developmental, mobility aids, etc.
  • Complete and maintain current and accurate documentation related to assessment findings, care plans, progress and discharge summaries according to CAR, the referral source and College of Physiotherapists of Ontario standards
  • Develop personalized, evidence-informed care plans to support individual rehabilitation and wellness goals.
  • Collaborate with a multidisciplinary team to coordinate patient-centric care and achieve optimal outcomes.
  • Build the next generation through mentorship and leadership (as assigned).
  • Stay current with best practices through continuous learning and professional development.
  • Utilize modern tools, including e-charting and digital communication, to streamline your workflow.
  • Partner with therapy assistants and kinesiologists to maximize clinical impact.
  • Educate patients and their families on recovery, prevention, and self-management strategies,
  • linking to community agencies/resources when appropriate.

  • Active license in good standing with your provincial regulatory college.
  • Professional liability insurance (as required).
  • Exceptional interpersonal skills and a patient-first mindset.
  • Enthusiasm for mentoring, participating in clinic events, and sharing knowledge.
  • 1–2+ years of outpatient clinic experience (preferred; new grads with provisional licenses are encouraged to apply!).
  • Ability to thrive in a collaborative, team-oriented environment.
  • Reliable transportation is required for this position
