
MSW-Social Worker

- Provides psychosocial and family system assessment, treatment recommendations, referral and discharge planning for children and adolescents in response to the unique and complex needs of this population
- Triage referrals from community partners including family physicians and pediatricians to help ensure the right patients are seen at the right place at the right time
- Able to adapt model of care interventions, including working within a 1-3 session model, provide risk assessment, crisis management and recommendations, referral and discharge planning for children and adolescents in response to the unique and complex needs of this population
- Actively involved in family education initiatives aimed at enhancing coping and reducing distress
- Works effectively with members of the multidisciplinary team in establishing and monitoring processes, improving continuity of care for patients, and enhancing collaborative relationships with other care providers
- Participates in service committees and quality improvement initiatives as required
- Engages in professional development and continuing education
- Assists with program development and evaluation as required Participates in team building, service development and improvement activities striving to contribute to a healthy workplace environment
- Other duties as assigned
- Master of Social Work degree with registration in good standing with OCSWSSW
- Clinical experience in the assessment and treatment of children and adolescents with mental health disorders, preferably within a hospital based outpatient service setting
- Demonstrated knowledge of best practice guidelines pertaining to the assessment and treatment of child and adolescent disorders
- Demonstrated skill in mental health assessment and crisis management
- Demonstrated understanding of and commitment to WRHN Mental Health & Addiction Program comprehensive safety programs and practices including staff, patient and environmental safety
- Proven ability to work effectively with other health care providers and community service and school board providers
- Excellent organizational, prioritization and problem-solving abilities
- Proven to have strong interpersonal skills and written and verbal communication skills
