- Contribute in delivering person-centered care by promoting the abilities and self esteem of the residents, develop strategies to promote optimal quality of life for the residents with the responsive behaviors.
- Conduct assessments using various assessment tools which describes behaviors of residents including observational behaviors, identifying behavioral risks, and identify caregiver strategies.
- Ensure the plan of care is regulated with responsive behaviors of residents in regards to the staff providing care and mitigating risks by conducting weekly rounds.
- Communicate with the external stakeholder and management team to identify the risks associated with responsive behavior.
- Demonstrate participation in quality improvement activities and provide monthly reports.
- Provide coaching and mentorship to the staff to ensure collaboration of care and services for the responsive behavior.
- Conduct resident care reviews and attend quality program meetings including P.A.C.
- Collaborate with the pharmacists and physicians for the consultation regarding the affect of medication treatment on residents.
- Participate in the staff development programs and seminars, and require to attend ongoing training to support skills in managing residents with responsive behaviors.
- Assist in tasks by considering policies of the home and the behavioral support Ontario project.
- Maintain amazing relationship with all the stakeholders including staff, residents and the general public.
- Provide an excellent customer service to all the stakeholders and assist in the administrative duties of the home.
- Other duties as required.
- Appropriate current certificate of competence from the College of Nurses of Ontario.
- Minimum two years of experience required in the similar role, preferably in the long term care setting, with comprehensive knowledge of duties.
- Degree in human psychology, or behavioral sciences would be considered as an asset
- Bilingual in English and French is an asset but not required.
- A serving heart and a passion for caring for seniors.
- Registration/eligible for registration with the Registered Practical Nurses Association of Ontario.
- Ability to work individually and as part of a team, to prioritize tasks in order of urgency.
- Excellent communication (verbal and written) and comprehension skills.
- Developed creative problem-solving skills to look at the “big picture.
