- Provide ministry funded, community based Occupational Therapy services within the client’s place of residence and/or through virtual services
- Provide occupational therapy assessment (through interview, standardized and informal assessment techniques) in the areas of home safety, home modifications, equipment prescription, fall prevention, mobility, cognition, stroke, etc.
- Develop and implement best practice and evidence-based occupational therapy intervention in collaboration with clients/caregivers
- Assign patient care plan, and provide supervision and monitoring of occupational therapy assistants when appropriate
- Effectively communicate information by providing education to client/caregivers and linking to community agencies/resources when appropriate
- Complete funding applications as applicable
- Complete and maintain current and accurate documentation related to assessment findings, care plans, progress and discharge summaries according to CAR, the referral source and COTO standards
- Opportunity to service private clients
- Registered Occupational Therapist with the College of Occupational Therapists of Ontario (required)
- Experience working within a community setting is an asset but new graduates are welcome
- Ability to work independently and as part of an inter-professional team
- Ability to prioritize, manage self-scheduling and meet reporting deadlines
- Excellent oral, written and report writing skills
- Proficiency with computers
- Strong interpersonal and organizational skills
- Reliable transportation is required for this position
