- Manage the delivery of coordinated quality care programs in designated long-term care facilities
- Work within the full scope of practice and will autonomously diagnose, order and interpret diagnostic tests and prescribe pharmaceuticals within the NP legislated scope of practice to avert transfers to emergency departments and hospital admissions
- Provide patient assessment, diagnosis, treatment, pain and symptom management to clients.
- Develop collaborative relationships with long term care home staff, Ontario Health atHome Care Coordinators, physicians, hospital staff and others
- Manage complex and difficult patient issues which cannot be handled in a routine manner
- Collaborative consultation with physicians to maintain patient centered care
- Participate in and provide effective advice and guidance to staff to help manage difficult and unusual cases
- Support Ontario Health atHome initiatives and implementation into practice, including Behavioural Supports Ontario
- Support data collection and program review consistent with NPSTAT objectives
- Design and implement material and strategic for enhance of clinical competency and capacity of long term care staff to address clinical complexity in the long term care environment
- Mentor and assist in orientating new NPs and support other clinical programs as directed by NP leadership

- Master’s degree, any relevant field
- Current RN(EC) registration with the College of Nurses of Ontario (with an NP certificate in Primary Health Care or Adult Care)
- A minimum of 3 years nursing RN experience
- Working knowledge of Ontario Health atHome’s role in the long term care sector
- A true passion for making a difference in the lives of our long-term care residents
