This role upholds the dignity and individuality of each person by recognizing and honoring their unique cultural, spiritual, and psycho-social backgrounds, fostering an inclusive and respectful environment.Person-and family-centered care (PFCC)Person-and family-centered care (PFCC) is an approach to care that recognizes the importance of family in a patient’s life and the importance of their active involvement in planning and making decisions on health care, services and treatment, and health system reform. This approach to care provides respectful, compassionate, culturally safe, and responsive care that meets the needs, values, beliefs, and preferences of the patient, their family, and others identified as significant to their life from diverse backgrounds and settings.Part-time position available: Requires at least 2 days of weekday daytime availabilityLocation: Markham, Stoufville and Uxbridge (with work available in the surrounding areas of Markham)Community based position, pay per visit and mileage in-between clientsDuties and ResponsibilitiesConduct assessments and building programs to reflect patient needs.Facilitate individual activation programsPlan appropriate programs for patients with dementia in collaboration with their family/caregiver when requiredUtilizes activities that promote cognitive function such as puzzle games, trivia, music therapy, etc.Incorporates technological integration when interacting with patients, as appropriateDemonstrated ability to work independently and in collaboration with others and as part of an inter-disciplinary team.Manages assigned budget and resources efficiently, benefiting both the program and patientsConduct short surveys on a regular basis at every session to track all clients’ satisfaction, using the data to further improve the programParticipate in ongoing internal and/or external continuing education activities.Adhere to Bayshore Policies and Procedures.Participate in quality activities and continuous improvement initiatives in keeping with the company's Quality Management System.Participates in proactive Health & Safety activities while performing all duties. Is responsible to notify immediate Supervisor of any Health & Safety risks or concerns.Maintains confidentiality of client and corporate information and discusses same only with appropriate Bayshore personnel.Complete other tasks as requestedQualificationsEducation RequirementsDiploma in Recreation and leisure studies, therapeutic recreation, kinesiology, or equivalentGentle persuasion or Montessori preferredCPR/ First Aid preferredOther Requirements:Personal vehicle and Drivers LicenseExceptional command of the English language as required for documentation and oral communicationsCurrent criminal reference check and vulnerable sector screenDemonstrated basic knowledge of various software programs including word, excel and power point and proficiency with emailExperience:Minimum 3 years of recreational coordinator/activation experiencePrior experience conducting assessments and building patient focused programsOther Skills and AbilitiesEnjoy working with clients of all ages and seniors in particularReliability, time management and the ability to prioritize workload is crucial.Creative, energetic and approachable with exceptional interpersonal and decision-making skillsAbility to handle difficult situations in an objective and consistent manner.Commitment to continued learningBayshore is committed to a high quality and safe environment for both our staff and clients/patients.
