In this role, you will provide high-quality, client-centered rehabilitation services to individuals in the community while supporting a new hospital-to-home care initiative designed to improve the transition from hospital to home.Working within a collaborative and interdisciplinary care model, you will partner with a diverse care team to support clients in achieving their health and recovery goals. JOB RESPONSIBILITIES:
  • Conducting assessments
  • Formulating goals and recommendations
  • Planning and implementing client-centered, goal-directed treatment programs
Requirements
  • A graduate of an accredited program in Physiotherapy and are a member in good standing of the College of Physiotherapists of Alberta.
  • Recent experience in managing patients in acute/rehab or community-based rehabilitation programs is preferred
  • Reliable, flexible, creative, self-directed professional, able to work effectively and collaboratively with rehab assistants as well as with a high-performance interdisciplinary rehabilitation team
  • Sound knowledge of community resources is an asset
  • Effective interpersonal, organizational and communication skills
  • A positive work and attendance record is essential
  • Ability to adapt to new computer applications
  • This role follows a hybrid work approach where you are able to work remotely and with the clients in their acute/rehab or community-based rehabilitation setting
  • A vehicle and valid Class 5 driver’s license and the ability to travel
