  • Provides physiotherapy assessment, consultation and planned interventions for client/patients
  • Implements treatment plans based on assessment, client/patient input and in conjunction with the client/patient’s integrated care plan
  • Documents assessment findings, treatment plans, and progress and discharge summaries of interventions as required
  • Regularly assesses the client/patient’s surroundings to ensure that client/patients have a safe home environment e.g. recognize fall risks, hoarding – reporting concerns to the Program Manager and/or Funder
  • Observes the performance of those assisting in the physiotherapy program and provides guidance as necessary
  • Adheres to the code of ethics and maintains professional standards of practice as defined by the College of Physiotherapists of Ontario
Qualifications
  • Degree in Physiotherapy from a recognized educational institution
  • Currently registered and in good standing with the College of Physiotherapists of Ontario
  • Proficiency in English both written and verbal; additional languages an asset
  • Demonstrates proficiency in the use of technology (MS Office, ability to learn proprietary software)
  • Current police check for work with the vulnerable sector
  • Valid G2/G driver’s license, third party insurance coverage at $1,000,000.00 and must have access to a reliable vehicle
