Closing Date: April 16, 2026Position SummaryThe Pharmacist provides comprehensive pharmacy services to the inpatient Multi-Organ Transplant Unit within the Ajmera Transplant Centre, with cross-coverage to other areas as required.DutiesAs a member of the patient care team, the principal responsibilities include:
- Conducting best possible medication history (BPMH) interviews and performing medication reconciliation
- Reviewing and dispensing all doctors' medication orders for appropriateness
- Identifying, resolving and preventing drug therapy problems with the patient and health care team
- Monitoring drug therapy for effectiveness, toxicity and adherence to evidence-based medicine/guidelines
- Following-up on medication incidents and promoting safe medication practices on the unit
- Responding to drug information questions; providing patient education; facilitating hospital discharge; and providing direct and/or functional supervision of technician staff
- Facilitating and supporting clinical teaching and research activities within the department, while exemplifying UHN's values
- Possess a Bachelor's of Science Degree in Pharmacy, Doctor of Pharmacy degree or equivalent required
- Certificate of competency from the Pharmacy Examination Board of Canada required
- Active registration with the Ontario College of Pharmacists (Part A) required
- Hospital pharmacy practice experience with patient-centered care experience is a requirement.
- Hospital Pharmacy residency an asset
- Membership in the Canadian Society of Hospital Pharmacists and/or other professional societies an asset
- Experience in an inpatient acute care setting is required
- Experience in an organ transplant care setting is an asset
