Nurse Practitioner - Main Street Project
Winnipeg, ManitobaWe are seeking a Nurse Practitioner to join the team at Main Street Project. Reporting to the Director of Healing and Harm Reduction, this role entails delivering primary health care services to Main Street Project community members, encompassing independent nursing services, continuous health management, administration of prescribed treatments, collaboration within the healthcare team, and coordinating care, with a primary focus on Withdrawal Management Services. Additionally, the Nurse Practitioner will offer guidance and support to Main Street Project's harm reduction programs.The Nurse Practitioner is Responsible For:
  • Performing comprehensive health assessments using sound clinical, judgment, diagnostic and therapeutic skills to evaluate patients' physical and mental health.
  • Conducting patient interviews, physical assessments and take medical histories and advise community members on health care including health promotion, disease, illness and accident prevention.
  • Implementing strategies to promote health and prevent illness with community members.
  • Consulting with physicians or other health care professionals when the patients' condition requires care beyond the scope of NP practice, and managing information database.
  • Formulating and implementing treatment plans and monitoring patient progress.
  • Providing care to community members in the Withdrawal Management Services program including; reviewing medial forms, monitoring prescriptions, providing health checks, and monitoring during stay.
  • Performing comprehensive health assessments using sound clinical, judgment, diagnostic and therapeutic skills to evaluate patients' physical and mental health.
  • Training staff to adhere to the highest standards of medication administration.
  • Acting as the patient advocate showing respect, dignity and consideration for community members.
You and Your Experience:
  • Master's degree in nursing; or completion of a nursing program with additional advanced or extended primary health care nursing education; and relevant practice as a Nurse Practitioner. Registration as an NP within the province of Manitoba is required, ensuring work aligns with guidance from the Manitoba College of Nurses.
  • CPR/AED Level C & Cleared criminal background check vulnerable sector search certified.
  • Must be able to meet physical demands of job, ability to lift up to 50 lbs.
  • Advanced written and verbal communication skills and displays cultural intelligence.
  • Knowledge and ability to apply the principles of Truth & Reconciliation.
  • Knowledge in health promotion, community development and community needs.
  • Must have drivers license and attend inter-agency and program related meetings.
  • May be exposed to infectious waste, communicable diseases/ conditions etc.
