Closing Date: March 25, 2026The Behaviour Support Worker is a member of a multidisciplinary Behavior Support Service (BSS) team, whose role is to help older adults with responsive behaviours have a successful discharge from acute care settings. The Behaviour Support Worker will travel between hospital sites and provide support to facilities within the BSP catchment area.Duties
- Collecting patient behaviour information through direct observation, caregiver and support interviews and other standardized assessments
- Working collaboratively with the BSS Mental Health Clinicians on the team to contribute to a behavioural care plan
- Implementing behavior support strategies with patients
- Keeping accurate clinical and operational records, as per policy
- Providing education and support to care teams
- Providing transitional support to LTCHs.
- Completion of a 2 year community college program in behaviour sciences, mental health studies, social service worker, gerontology or recognized equivalent
- One (1) year related practical experience supporting patients in LTCH, acute care and/or mental health care setting
- Completion of P.I.E.C.E.S., GPA, U-FIRST and Dementiability training considered an asset.
- Knowledge of applicable LTCH, mental health and hospital legislation and policies.
- Sound understanding of and commitment to the principles of the Behavioural Supports Ontario Project.
- First Aid and CPR certification
- Excellent organizational and time management skills
- Excellent interpersonal and communication skills
- Exercises initiative, flexibility and good judgement with competing priorities
- Ability to produce high quality work in accordance with Hospital standards
- Ability to work well under pressure
- Ability to use good judgment in assessing difficult situations
- Access to a reliable vehicle and valid driver's license, an asset
- Eligibility for membership in relevant associations/professional organization considered an asset
