
Intensive Case Management Worker (CP-Wknd) - St. Josephine

- Assesses dangers, develops safety plans and participates in collaborative risk management to support clients in obtaining or maintaining safety
- Provides coordination of informal and formal support systems and coordinates comprehensive wrap-around community services for clients such as healthcare, legal, training/education, life skills, translation and other needs.
- Identifies objective goals with clients to plan and develop a service plan & regularly monitors and reviews the progress of the goals that each client has set
- Provides a safe, nurturing environment for clients & provides consistent support around specific life and social skills to increase client capacity and skills
- Supports clients with system navigation, communication and conflict resolution through role modelling and coaching to assist clients in fostering their independence, resilience, and self-determination as appropriate to their abilities and experience
- Liaises with community partners to provide culturally safe programming and support services.
- Participates in daily household and operational duties, including meal preparation, cleaning, and sanitizing.
- Provides crisis intervention and problem solving support to any emergency situations
- Works from a trauma-informed, low barrier, client-centered approach ensuring clients feel valued and supported.
- Understands, supports, promotes, and adheres to the Agency’s vision, mission, values, and organizational ethics, treating all persons with humility, compassion, and respect in interactions with clients, stakeholders, and the public
- Maintains respectful personal boundaries
- Establishes and maintains respectful, collaborative working relationships with all team members, clients and their key stakeholders. Presents as a positive and professional role model and ambassador for the Agency.
- Communicates effectively and respectfully in a professional manner with clients, both verbally and in writing. Actively listens to clients to understand their needs while being aware of and responding to non-verbal communication.
- Documents case notes and updates/maintains files and records data, ensuring the data is accurate, timely and complete. Ensures appropriate and confidential handling and storage of client/Agency information and documentation in accordance with Agency policies and procedures
- Completes incident reports, serious occurrence reports and/or other administrative duties in a precise and timely manner as per Agency policy and procedures
- Attends and actively participates in regular supervision, scheduled team meetings and professional development.
- Minimum of a Diploma in a Human Services’ discipline or equivalent
- Degree in Social Work is an asset
- Additional relevant certifications or designations considered an asset
- Minimum of 2 years’ related professional experience
- Equivalent combinations of education and experience may be considered
- Police Information Check including vulnerable sector search, and Intervention Record Check current within six (6) months
- Enrolment in the Agency Group Benefits plan as applicable
- Satisfactory driver’s abstract with no more than six (6) demerit points current within one (1) year.
- A reliable Vehicle, Valid Driver’s License, Vehicle Registration and Third Party Liability Insurance (amount of two million)
- The Annual salary for this position is $19,889.12 - $22,383.86 (weekly 16 hours) based on qualifications and experience. We offer flexibility and supportive working environment.
