- Is the liaison between the and Ontario Health at Home Placement Services
- Oversee the management of applications on HPG for interdisciplinary review, ensuring timely scheduling and facilitation of admissions.
- Monitor waitlist and provide education and communication to those listed
- Provide tours for potential residents and families
- Educates team members prior to resident moving in on residents history and current needs. Ensure interventions in place prior to arrival.
- Is point of contact on day of admission ensuring contact with interdisciplinary team.
- Supports residents and families through the move in process for multiple weeks
- Coordinates Admission Feedback Survey.
- Ensures documentation for admission is complete and accurate in accordance with legislation and homes policies
- Help prepare residents and their SDMs both before and at arrival to ensure they are comfortable and understanding of the admission processes.
- Help personalize the resident's experience. Support team members knowledge of residents life experiences and needs. Assist resident to engage with new residents and support their SDM's to ensure a smooth transition
- Coordinate and facilitate admission and annual care conferences, collaborating with a multidisciplinary team to provide comprehensive support for residents as needed.
- Ensure follow up is completed for care conference action items
- Act as Homes liaison for Resident Council. Ensure follow up for concerns and suggestions made.
- Facilitate family orientation sessions quarterly
- Act as Homes Family Council liaison
- Fostering communication through one-on-one interactions, and surveys
- Relay any concerns or issues to proper department or manager for prompt resolution.
- Connect family with external resources to support specific resident or family member related concerns.
- Provide psychosocial support to resident and family members during any time of transition or significant change.
- Collaborate with the home to uphold resident care standards, ensuring that residents' needs are identified and addressed effectively.
- Is a member of the Continuous Quality Improvement Committee and participates in review and development of TCMH policies and procedures, as well as Quality Improvement Plan and other Quality initiatives.
- Assists in ensuring residents are free from abuse, mistreatment and neglect. Follows mandatory requirements as per the Fixing Long Term Care Homes Act of 2021.

- Supports the home concern and complaint process.
